This is a typical lament of the newly-graduated nurse, looking for his or her very first job out of school, at least in some parts of the country, and in some situations. I believe that some encouragement is needed, as well as some "sage advice."

The job market, in some places, is very tight. While it is disheartening, we need to realize that this isn't completely new. Nursing, as a profession, has been here before, to a degree. When I first began my career, nurses were being laid off, allied professions were being cut...this was over 2 decades ago now. New grads and both current and time to come nursing students: You're caught in the middle of a genuinely weird situation right now. Trust me...there is a nursing shortage! And it is going to get worse.

The problem seems to be that, like every other firm around, hospitals are having to make the same gut-wrenching budget cuts as everyone else. It's hitting so many areas of nursing right now...students, faculty, schools, hospitals...everyone is affected by the current economic situation. Hospitals, either they're short on nurses or not at the moment, are dealing with a cash-crisis. A brand new nurse, fresh out of school--no matter how many "A's" you got in nursing school, no matter how many articles you've written above and beyond, no matter how many volunteer/student-work/extra-credit hours you've logged--a brand new nurse will take close to a full year to mentor and precept into an independent Rn. They will spend tens of thousands of dollars on you, above and beyond the wages they pay you, just to get you to the place where you genuinely "earn" that salary. Don't be offended...the hospital typically knows that you are a great investment! These just aren't typical times right now.

You may not believe it right now, but most of the skills of nursing are learned after you get out of school! In school, you are studying the "science" of nursing, the "theory" of nursing. Upon graduation, you will learn how to apply that science and law in the real world of nursing. Your clinical rotations were not the real world. Nursing requires judgment skills; judgment skills are the follow of sense backed by the law and science you learned in school. It just takes time.

Ok, so...what can you do? First, identify that you Do have options:

1. realize that your first job is just that...it's your first job. Few new grads, either they're nurses, lawyers, engineers, or architects, land their dream job right out of school. When you say that there are "no jobs anywhere" in your area, is it genuinely No jobs? Or have you dinky yourself in any way by not considering jobs in, shall we call them, "less than desirable" specialties? I genuinely disliked my first year of nursing! But you know what? It was only my first year. Once it was over, I was the "experienced Rn" that hospitals were crying out for. I named all my time to come positions, where and when I wanted them. But that first year, in what amounted to a "glorified nursing home" was not what I had Ever imagined for myself. So...have you genuinely looked everywhere?

2. I have read more than one nursing pupil posting comments online about how upset they were that there were "No Jobs" out there, only to then read that she is a senior in nursing school or a brand new graduate nurse who wants to go on to become a nurse anesthetist, and to get into that agenda she has to have at least a year of Er or Icu experience...and "no one will hire me." To such students and grads, may I tell you in the kindest way that if any hospital does hire you into their Er or Icu as a new grad, they are setting themselves...and very maybe you..up for a inherent lawsuit because of the dire consequences your lack of sense and teenage expert judgements may cause someone?

I worked 10 years of my work in significant care...All areas of significant care...and new grads plainly do not have the knowledge, skill, or judgment abilities to work in these areas. Period. Want to become a Nurse Anesthetist? Then graduate nursing school, take anything job you need to to get working as a nurse, so you can genuinely begin to function as a "real" nurse (not just a pupil nurse!) at the bedside, fulltime. Learn. Learn all you can in that first job. Be the best new nurse you can be.

Get the best peer reviews. Get the best reviews from your Unit Manager. Be the nurse the patients and their families write letters to the hospital directors about (good letters, of course)! Then, at the end of that year, go apply for a job in the Er. Go get a spot in the Icu. Believe me, when you're in there, you'll be starting all over again with the studying curve! But when you're in, you're in...now, remember what you did that first year in that first position? Do it again. At the end of that year, go apply for that slot in the Nurse Anesthetist program. Smile...you'll have earned it, because you worked for it. Well worth it!

Again, few new graduates, anything their profession, land their "dream job" fresh out of college. Most new grads expect to start, oh, somewhere near the bottom, and work their way up, gaining experience, wisdom, and leadership skills along the way that will be used in their futures. In nursing, we are fortunate...the lowest isn't that far from the top. It doesn't typically take more than a year of doing what you'd rather not be doing in order to shoot straight to where you do want to be. So just get started.

2. Let's say you genuinely have looked at every hospital, every nursing home, every assisted living center in your area, and there are No jobs. You have a decision to make. I tell my own kids this all the time: you can either pick where you want to live, and then work at anything you like best that is available there, or you can pick what you'd love to do, and then go wherever you have to in order to do it. It's just that simple. With a work in nursing, If you wait long sufficient and are willing to do what it takes at first (probably not too long, but be ready for a year or so), you'll probably be able to have Both.

Jobs Are out there. Go where they are, get your feet wet and become the experienced, independent Rn everyone's looking for! Do what it takes! It's Worth It!

First of all, as a trainee nurse you were given assignments that were not beyond the scope of your convention or ability. In expanding to that, you were working under your instructor's license and whether or not you were aware, they kept a very close watch on you. You are out there by yourself, free to make decisions and make mistakes. With those decisions and mistakes you are also, Fully Accountable.

Now fantasize yourself with eight or more patients. You may be asking yourself, "What do you mean eight or More patients?" Most trainee nurses never have more than six or seven patients and that is only at the very end of the program. Even then, the trainee nurses can delegate tasks to assistive personnel thereby lessening their work load. However, there will be time that you will be on a floor, without any assistive personnel, with more than eight patients and no one to rely on except yourself. To blend this scenario, there may be patients that have very essential care pathways that you must follow, incoming calls from patient's families, doctor's, operating room staff, and other collaborative care departments requesting information.

Now, let's stop this scenario in its tracks. You probably feel the stress construction by just reading the previous passage. If so, don't worry, it is a perfectly general reaction. So, relax, and know that there are ways to ease this safe bet right of passage. Let's take a look at some of the ways to help ease and deal with reality shock.

1. First of all, it is leading to understand that, "reality shock" does easily exist and you will come face to face with it. Knowing what you will face is most of the battle. The task at hand then becomes, knowing what steps to take and resources to use, and how to use them.

2. Learn as much as you can during your in-service training program, which usually lasts up to eight weeks or more. This means familiarizing yourself with every inch of your floor. Know where everything is located, from security pins to I.V. Tubing. You may never know when you may need it, Now, and the worst thing that can happen is that you spend high-priced time looking for something as small as a security pin.

3. Know, who the assisted personnel are, and know them by name and face. Find out who will be the ones that you will be working with during your shift. What usually happens during in-service training is that graduate nurses train on the main shift, which is most oftentimes days, before transferring to the shift that they have been hired to work. Therefore it is very leading to know who you will be working with during your shift. Try also to establish a working connection with the assistive personnel. Remember, it is not a popularity game, you are not out to make best friends. Your goal is potential patient care, and the patient comes first.

Let your intentions be known that your number one goal is to give the best patient care that you have respect for the care team assistive personnel and are a willing and helpful body to them as well. In doing this, you will have disabled the imperceptible wall that exists between nurses and assistive personnel. That wall is the one entitled, I am a Nurse and you are Only an aide. One thing that you must strive for is to use an even tone of voice, and fairness when delegating. You must also remember the proprietary of delegation, which are as follows,
a. The right task,
b. The right person,
c. The right communication, (must be clear and concise),
d. The right feedback, (the man who is delegated the task must comprehend what it is that is asked of them and let the nurse know that they comprehend). By doing this you are ensuring that patient care will not suffer do to a personality glitch, which could have been avoided.

4. Know, know and Know where the procedure and procedure manual is settled on the floor. Part of your hospital in-service will contain the introduction of the procedure and procedure Manual. This is the manual that you will have to refer to many times for protocol from everything from changing out a Foley Catheter to transporting a patient to an additional one floor. The final determination will be in any investigation, "did the nurse use and result the procedure and procedure Manual?" You want to all the time be sure that you result the rules and protocols contained in your institutions manual. Therefore, you should know where it is settled and be familiar with how to look up varied procedures and policies.

5. The procedure with regard to medication errors. Most everyone makes them and it is crucial that you know what paperwork is required to be filled out. It will come in handy. You cannot just, "wing it", when manufacture a decision, you have to result protocol.

6. Use assertive transportation when interacting with doctors'. Assertive transportation indicates that you are aware of yourself and your limitations as well as your liability to the patients that you care for. 7. Ask questions. You will have time during your in-service training to ask questions and get answers. It is your right to do so. Remember, not knowing is not an excuse, and you do not want to be in a situation where there will be no-one to ask. This is not to say that you will know everything, but a least you are giving yourself a head start and a good solid foundation by knowing what you can. So don't be afraid to ask. Also, know who your resources are, for questions that you may have on the shift that you will be working.

8. Find a mentor with whom you can relate. Try to find man not only on the shift during training, but also on the shift that you will be working. It is nice to find man who has the caress and understanding as well as man that you can get along. There might be a time that you will want to call them in the middle of the night and vent your concerns, or just to have them give you safe bet input in your performance. Mentors are essential to the increase of a new nurse.

9. Lastly, Know, Know and Know, what your State Nurse convention Act states. This Act, is your guideline for most all that you do as a Registered Nurse. Know it well.

Hopefully these tips will help to ease the reality shock that you will face during your transition from trainee nurse to full time graduate nurse. Remember, you can't do it all.
